POSITION SUMMARY:
The Occupational Therapist under the direction of the Manager and/or
Director of Rehab Services, functions as a clinician/specialist in
assigned area of specialty within the Occupational Therapy Department.
The therapist will perform an occupational therapy evaluation and plan
and direct subsequent treatment based on assessment findings and/or
referral of physician. The therapist is responsible for collecting
pertinent patient information regarding each patient’s level of
independence in ADL’s, need for adaptive equipment, motor skills and
functional cognitive abilities to provide appropriate therapeutic
interventions. The therapist documents assessment findings and
subsequent treatments in a timely fashion. The therapist provides care
within the job scope and in accordance with Ministry Healthcare’s
mission, standards of care, policies and procedures.
ESSENTIAL JOB FUNCTIONS or RESPONSIBILITIES:
• Collaborates with the patient care team by problem solving, working
cooperatively, and assuming responsibility for appropriate patient care
activities and interventions.
• Provides assessment of current function, plans for treatment,
evaluates progress toward goals and provides necessary education to
assist patients in preparation for discharge.
• Assess patients’ status through interview, observation and physical
evaluation.
• Provides evidence of contribution to the improvement of patient care.

QUALIFICATIONS:
• Education: Bachelor’s degree or Master’s degree in Occupational
Therapy from an accredited course of study is required
• Experience: No experience is required, one year of experience is
preferred
• Licensure: Licensed as an Occupational Therapist by the State of
Wisconsin
• Skills: Assess data and identify patients' age-specific requirements
and provide age-appropriate care
