Scholarship Award Application
Purpose
To recognize a person(s) who has made a significant contribution to the
field of occupational health in healthcare, and demonstrates support of
AOHP.
Qualifications
Members of the Association are not eligible for this award.
Procedure
- The executive president solicits nominations by way of an announcement
to regional directors and chapter presidents and via the Journal.
- Chapter presidents solicit membership nominations from chapter members.
- Chapter presidents and regional directors submit nominations with
a statement of qualifications and outstanding contributions to the executive
president.
- Nominations close August 15th of each year.
- The executive president submits a qualified list of candidates and
accomplishments to the executive board of directors for vote prior to
the October executive board of directors meeting.
- The awardees is announced at the annual membership meeting and is
presented with a plaque.
- Membership is awarded for one (1) year.
- No more than five (5) honorary memberships shall be conferred in
any one year.

Scholarship Award Application
Purpose/Qualifications
A $500.00 scholarship to be used for educational purposes at the discretion
of the recipient (classes, conferences, subscriptions, library, etc.)
shall be awarded annually to a past or present Association officer who
best portrays an occupational health professional in healthcare role model.
Procedure
- The executive president solicits nominations via announcement to
the executive board of directors and chapter presidents in May. Submit
nominations to the executive president, denise.strode@osfhealthcare.org.
- Nominations close August 15th of each year.
- A list of candidates and their qualifications shall be presented
to executive board of directors to be completed by October 1st.
- The awardees shall be announced at the annual membership meeting.

Scholarship Award Application
Purpose
To encourage, promote, and strengthen the knowledge base and expertise
of the occupational health professional in healthcare.
Qualifications
The Association of Occupational Health Professionals in Healthcare invites
proposals for an original research project on current and/ or anticipated
issues in healthcare-related occupational health. Proposals from nonmembers
are welcomed.
- The Research Scholarship Award is $2,000.00.
- The proposal is due on or before July 15.
- Upon acceptance of the proposal $ 1,000.00 is awarded.
- The remaining $1,000.00 is awarded upon presentation of the project.
The scholarship recipient shall agree to present the completed project
for publication in the Journal.
- Scholarships shall be awarded based on merit in accordance with the
evaluation tool.
- Completed proposals shall be submitted according to the format
outlined below. Please include a cover letter with your name and
address. Do not include your name or any personal identification
data on the proposal itself.
- Address completed proposals and correspondence to the AOHP executive
secretary, Research Scholarship Committee chairperson.
Procedure
The following outlines the information required in the research scholarship
proposal. Please follow the outline exactly. If you have no information
to include under any particular heading, please write “not applicable”
or “not available.” Each submission must be typewritten, double-spaced,
and limited to 1,000 words.
- Provide a formal title for the project.
- State the one category from the following list that best describes
the area the research project will address: employment examinations,
medical surveillance, immunizations, infectious diseases, employee health
records, work injuries, administration, marketing occupational health
services, or other healthcare-related topics.
- Briefly describe the impact/ significance of the research project
to the occupational health professional in a healthcare setting.
- List the objectives and goal of the research project.
- Describe the activities that will be implemented to achieve the goals
of the project, e.g., questionnaire. As appropriate, describe the target
population, e.g., clinical (nursing, etc.) nonclinical employees.

Scholarship Award Application
Purpose
To recognize an Association member who has demonstrated extraordinary
leadership.
Qualifications
Active Association member for a minimum of two years employed in the
field of occupational health in healthcare. Current executive board of
directors members and current chapter presidents are not eligible for
this award.
Procedure
- The executive president solicits membership nominations by way of
an announcement to regional directors, chapter presidents and via the
Journal.
- Chapter presidents solicit membership nomination from chapter members.
- Chapter presidents submit membership nominations with a statement
of qualifications and extraordinary accomplishments to the executive
president.
- Nominations close August 15th of each year.
- The executive president submits a qualified list of candidates and
accomplishments to the executive board of directors for vote prior to
the October executive board meeting.
- The awardees is presented with a plaque at the annual conference.
A letter of recognition shall be sent to the recipient's CEO from the
executive president.

Scholarship Award Application
Purpose
To provide annual continuing education scholarships to subsidize the
educational efforts of members.
Procedure
Funding Guidelines
- maximum of $600 per member shall be awarded annually.
- Monies shall not be awarded in conjunction with other Association
scholarships for the same course.
- Applications for scholarships to the Association's annual conference
must be submitted by July 15th of the year of the conference. The maximum
award is the amount of the conference fee.
Award Criteria
- Applicant must be an AOHP active member in good standing.
- Award limited to one (1) per member, per year.
Process
- Submit an application by July 15th . Address the application to the
executive vice president and include:
- Application form
- Conference brochure (brochure not needed for AOHP conference
applications)
- The executive vice president shall notify the applicant in writing
of the final decision.
NOTE: Deadline for scholarship applications for the AOHP annual conference
is July 15.

Scholarship Award Application
Purpose
To recognize a chapter which has demonstrated outstanding performance
and enhanced the image of employee health professionals. The chapter recipient
shall be awarded $250.00 and a certificate of appreciation at the annual
conference.
Qualifications
Chapter must be an active Association chapter for a minimum of two years.
Applications must be received by July 15th of each year. To be eligible
for this award, one chapter member must have been published in the AOHP
Journal as per the instructions.
Procedure
The chapter president shall apply to the executive vice president, utilizing
the approved application form.
Include information for current year and year prior to application. Please
submit (typed) the following material with the application form:
- List chapter goals, objectives and achievement status.
- List chapter members who have published in The Journal of the Association
of Occupational Health Professionals in Healthcare. Include article
title and publication date.
- List chapter members who have published in other professional journals,
textbooks, etc. Include title and publication source.
- List chapter members who have presented papers, posters, etc. at AOHP
sponsored conferences.
- List chapter members who have served as a speaker for AOHP sponsored
conferences.
- List chapter members who have held chapter office, executive board
office, or other AOHP sponsored task forces and activities.
- List other chapter or member activities indicative of chapter excellence.

Purpose:
To recognize business(es) that support the occupational health professionals and membership and participation in AOHP.
Qualifications:
Any health care organization, business, corporation, or government agency which actively supports occupational health professionals in healthcare and their participation in AOHP programs and activities.
Procedure:
- The Executive President shall announce the award nominations to the Executive Board and Chapter Presidents
- Nominations must be submitted by an active AOHP member
- Applications must be submitted by July 15th
- The Executive President and 2 other Board members shall review the applications
- More than 1 award may be made annually
Instructions:
Include information/activities for current year and year prior to application. Please submit (typed) the following material:
- Indicate appropriate company official to notify if selected for the award
- AOHP will notify all applicants in writing and will send a copy to the member(s) nominating the winner(s) prior to the national conference
- The winning submissions will be posted on the AOHP web site and announced in the Journal. A press release will be developed and sent to the business for local media recognition.
- Submit the original of a statement and attachments
- The statement and attachments should not exceed fifteen (15) 8½ x11” typewritten, double spaced, single sided pages with 1 inch margins and 12 point font. For all acronyms, provide the full name of the organization or program the first time it is used.
- Entries may be mailed, faxed, or e-mailed to AOHP Headquarters
